The primary responsibilities of the charge/church conference in the annual meeting shall be to review and evaluate the total mission and ministry of the church, receive reports, and adopt objectives and goals recommended by the church council that are in keeping with the objectives of The United Methodist churches.
These forms are provided to assist in preparation for your annual meeting however that happens (with the district superintendent presiding or at another time established by the congregation which also has been authorized by the district superintendent with an authorized elder presiding).
All forms are fillable PDFs with the exception of Form 2 (an online form) and Forms 5, 5a and 7 (can be filled either with Microsoft Excel or as fillable PDFs). Fill PDF forms using Adobe Reader (Windows or Mac) or Preview (Mac).
Those forms marked with an * are to be submitted to email@example.com. While other forms are to be completed, they do not need to be submitted to the above address or the district office.
Forms 5 and 7 are to be emailed to firstname.lastname@example.org by December 1, 2018. Any other forms required to be submitted to email@example.com are due two weeks after the charge/church conference is held.
