Clergy and conference staff with Wespath health insurance, it’s time to register for the Blueprint for Wellness Screening (health screening) available at Annual Conference Session.

You must register before May 11 for a time slot at conference session; walk-ins are very limited.

To register online: Log in to your Virgin Pulse account. Under “Benefits” tab, find Quest Blueprint for Wellness and select “START NOW”. Or call 1-855-623-9355, the employer group is HealthFlex or United Methodist Church. Register for the Minnesota Conference screening day on Wednesday, May 25 from 6 a.m. to 10:15 a.m. in the Alexander room.

Participant and spouse on the HealthFlex plan can each earn the $100 PulseCash incentive. (See flyer for more information) If you are unable to attend conference session for the screening, you can find information to complete the screening at a Quest Diagnostic facility online or on the call.  All screenings must be completed between January 4 and August 31 to receive the incentive. There is no cost to HealthFlex participants and results are mailed directly to you.
 
The Blueprint for Wellness results are mailed to you, and you can share the results with your doctor. To assist with the Health Check, the Blueprint for Wellness data will be imported to the Health Check for enhanced accuracy and ease of completion. Remember to complete the Health Check due by August 31 to avoid an increase to your deductible for the coming year; deductible increases are $250 for single coverage or $500 for employee plus one or family coverage. Both the member and spouse will need to complete the Health Check if both are on the HealthFlex plan to avoid the deductible increase.
